Your first visit to our involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. We also describe our first visit procedures. We recommend you save some time by printing out and completing the patient forms in advance of your appointment. PLEASE BE SURE TO BRING YOUR INSURANCE CARD TO EVERY APPOINTMENT . You are responsible for obtaining a referral from your primary physician prior to your appointment if one is required by your insurance carrier.

This is our new way of checking in patient prior to appointments, as well as eliminating the paperwork needed if completed prior to appointment. If you are unable to log in using this application we ask that you print the forms in the link above (patient registration). If you are unable to print the forms we require you to arrive to your appointment at least a half hour before your scheduled time to complete the forms. Failure to do so may result in your appointment being rescheduled.

To download the application

  • Go to your smart phone or tablet application store and download APPatient
  • From this application you can make changes to your medical history, update your patient demographics, check-in, make your copayment, and even send messages to our staff.
  • To log in you will need the following information:
    • Practice URL:
    • Username: Email address that you provided our office
    • Password: Patient will set this up themselves, if you have never logged in before please click forgot password and it will prompt you on how to get in for the first time.

Local landmarks
Traveling north on route 10, pass McDonalds on the right, go through one more traffic light (Higgins Road). Take the next left into our parking lot. Our office entrance is located at the front of the building all the way to the left, up the ramp and through the glass vestibule.

Traveling south on route 10, pass Cheshire High School on the right. Turn right into our parking lot in approximately 1/8 th of a mile. Our office entrance is located at the front of the building all the way to the left, up the ramp and through the glass vestibule.
